This is your Business Associate Agreement. In it, you will find a template of the form you should be using to ensure that any advisor or employee that you hire will agree to abide my HIPAA regulations. Remember, you can edit or add to any particular form at any time, so take this template and make them your own as you like. Or just use it as is, it's your choice!
Be sure to discuss any form with your supervision before using them for your business.
